New customers must submit an Application to begin receiving services through the MTMA. The application can be picked-up at the MTMA office, mailed or faxed to the new customer, or new customers can click on the link below to print a copy of the application and either mail or fax it back to the MTMA offices.
Per US Department of Homeland Security requirements, all customers that sign up for service must also submit a copy of their driver's license or passport with their application for identification purposes.

Residential Application
This application is for new customers that have purchased an existing house or newly built house without a fire protection system and are signing up for service with us.

This application is for new businesses that have either purchased property or are a tenant.

Deduct Meter Application
This application is for any residential customer that owns a house and wishes to install an irrigation system. There is a $200 fee and it is the owner's responsibility to have the backflow device tested annually by a certified inspector and to submit the report form in accordance with MTMA regulations. The deduct meter shall be purchased from MTMA at the authority office. The cost of the meter is not included in the application fee. Please call the office for meter pricing. Sewer will not be charged for water passing through this meter. However, owner is responsible for maintaining the meter. For a diagram on how the meter spread should be installed click here.

Rental Property Application
This application is for owners that have purchased residential property that they plan on renting out to others. Please Note: The property will always remain in the owner's name, not the tenant's name. All residential accounts must be in the name of the owner or a property managers account.
Application for Tenant Duplicate Billing
This application is for owners that wish to have their tenants receive a copy of the utility bill.
Applications for Developments and Tap Fees
With the water/sewer tap application, copy of the building permit application, plot plan, application for service, and tap fees must be submitted.

Application for ACH Enrollment
To make the payment process even more convenient, MTMA is currently enrolling interested customers in an automated draft bill payment system enabling them to have their monthly or quarterly bill payment automatically deducted from their checking accounts. Those interested can print out the Enrollment Form and either mail or deliver it in person to the MTMA Office at 1700 Beaver Grade Rd.
